Preventing a Mix-up
Eli Lilly and Company is committed to the safety of patients and ensuring that they are being dispensed the correct medications. Packaging for ZYPREXA® (olanzapine) has been changed to make identification even easier. All bottles of ZYPREXA have a tamper-resistant seal on the cap along with a revised bottle label.
Label changes include:
- Picture of tablet on label
- Tamper-resistant seal on cap
- Yellow highlights for "PREXA"
- Lower case "Y"
- Tamper warning printed in red

Recommendations on How to Reduce the Risk of Medication Errors
- Alert all pharmacists to the possibility of mix-ups when prescribing ZYPREXA and Zyrtec® (cetirizine HCl)
- When accepting telephone orders, repeat back all orders, spell the product name, and ask for the indication
- Highlight to clearly distinguish letters that are similar on storage bin labels
- Verify the indications with the patient
- Double-check the container with the prescription
- Encourage patients to ask questions about their medications
